ICARE Project Success Story: Empowering Adolescents Living with HIV for Improved Treatment Outcomes

This success story highlights the remarkable journey of Jabulon (not real name), a 17-year-old adolescent living with HIV in the Lolwe Islands of Namayingo District. Through the tailored support provided by the ICARE OVC program, Jabulon experienced significant improvements in viral load suppression and overall treatment outcomes. Jabulon’s struggle to suppress his viral load and adhere to treatment initially posed a significant concern for the clinical team at Singila HCII in the Lolwe Islands of Namayingo District. Recognizing the need for targeted support, Jabulon was enrolled in the ICARE OVC program in January 2021. This success story outlines the journey of Jabulon’s transformation, highlighting the interventions, partnerships, and measurable progress achieved through the ICARE project.

Tailored Support and Care Plan:
During the enrollment assessment, Jabulon’s challenges with viral load suppression and lack of parental support became evident. Being under the care of his father, who initially struggled to provide adequately for him, Jabulon faced interruptions in treatment, missed drug refills, and struggled to adhere to treatment prescriptions. To address these issues comprehensively, ICARE assigned a community Para social Worker to support Jabulon’s household in developing an evidence-based care plan.

Key Interventions and Milestones:
The care plan for Jabulon’s improved treatment outcomes encompassed several critical interventions:

  1. Enhanced Adherence Support: ICARE facilitated intensive adherence counseling sessions, ensuring that Jabulon received the necessary guidance and tools to adhere to his treatment regimen effectively. The dedicated community health workers attached to ICARE and LPHS conducted regular home visits, providing personalized support and monitoring Jabulon’s progress.
  2. Optimized Treatment Regimen: Based on the results of routine viral load tests in May 2021, which revealed a viral load of 3,427ml, ICARE made the necessary adjustments to Jabulon’s treatment regimen. The team ensured that he was on an optimal regimen that would aid viral load suppression effectively.
  3. Caregiver Involvement and Support: Recognizing the importance of a supportive environment, ICARE worked closely with Jabulon’s caregiver to provide the necessary support and education on his treatment. This collaborative effort helped establish a foundation of understanding, trust, and consistent monitoring.

Measurable Progress and Achievements:

Viral Load Suppression: Within just one year of enrollment, Jabulon achieved an undetectable viral load result by June 2022. This remarkable achievement signifies the successful suppression of the virus and demonstrates the effectiveness of the tailored care plan implemented by ICARE.
